Entry forms for the Paramotor event at ABG have been published by HABGOC.

It must be completed by:

  • Each participant
  • Each participant's 'National Federation' (NF) which is the participant's FAI member NAC.
  • Each Participant's National Olympic Committee (NOC)

The completed forms must be submitted to HABGOC by your NOC before 30 April 2012, 24:00 Beijing Local Time; UTC +8.

Instructions

It does not matter at what stage the Entry form is printed, but a competitor could fill out the form (as much as possible) and send it to their NAC, or a NAC or NOC could fill it out, print it and fill in the rest of it manually. In any case it must be printed at some stage to be be signed and stamped.

National Olympic committee Name The NOC should fill this out.

  • NOC Code The NOC should fill this out.
  • Family name
  • Given name
  • International Recognized Name(if any)
  • Passport No.
  • Nationality What it says in the passport.
  • Gender
  • Height(cm)
  • Weight(kg)
    Date of birth
  • Accreditation number Your NOC should know this.
  • FAI Sporting Licence number Issued by the NAC. This MUST be valid for Microlights and Paramotors for the period of the games and MUST be recorded as such in the FAI Sporting Licence database (if it is working).
  • Events IMPORTANT! All pilots should check all boxes. Since [the rules] expect all pilots to fly all tasks, by not checking some boxes they are only making themselves and their team less likely to win any medal.
  • Competitor's declaration See OCA Constitution and OCA Anti-Doping Rules. Commercial Guidelines will presumably appear on the official website at some stage before the event.
  • Competitor signature If you don't have a digital ID then this should be signed after printing in the normal way.
  • NOC Signature and stamp The NOC should complete this.
  • NF Signature and stamp The NAC should complete this.

The OCA Constitution Article 54 – Entries (see panel on right) says all pilots, team officials Etc. entering ABG must sign a declaration, and those declarations must be counter-signed by their National Sports Federations, which in this case is your NAC.

The deadline for receipt of these declarations has been set by HABGOC at 30 April 2012, 24:00 Beijing Local Time; UTC +8. (which is more than the 30 days required by the OCA constitution).

These forms should have already been distributed by HABGOC to your National Olympic Committee (NOC) and the completed forms must be submitted to HABGOC by your NOC.

NACs who have not already received these forms from their NOC should get in touch with their NOC immediately so they can be completed and returned to the NOC in good time for them to be submitted to HABGOC before the deadline.

3. Entry by Name

The NOC’s shall submit the entry by names of the Athlete and Officials to AGOC, at least thirty (30) days before the Opening Ceremony. All entries must be printed or typewritten in duplicate on a special form prescribed by the AGOC. Any variation on the deadlines should have the written approval of the OCA;

As a condition precedent to taking part in the OCA GAMES a competitor must be eligible as defined in these Rules;

The entry shall contain the following statement, which must be signed by all participants:
“I, the undersigned, declare on my honour that I am eligible and fulfil the Conditions stipulated by the OCA Rules and constitution. I am also fully aware of the OCA Anti-Doping Rules and the allowable trademark identification on clothing and equipment worn or used at the OCA GAMES as indicated in the Commercial Guidelines. Failing to comply with any of the above may result in penalties and my disqualification”;

The National Sport Federations of that particular sport must countersign this declaration stating that it is true to the best of its knowledge. The NOC shall sign the form and forward entries to the AGOC;

In the event of there being no National Sport Federations for a particular sport in a country, the NOC may act as the governing Sport body and enter individuals in that sport with the approval of the OCA;
